Florida CNA State Exam Application

Simple Step-by-Step Instructions (Prometric)


BEFORE YOU START (IMPORTANT)

Complete your Live Scan fingerprints FIRST

  • Required before submitting the CNA application

  • Cost: $100

  • Valid for 5 years

  • Must be healthcare-related (nursing home, hospital, CNA, etc.)

  • Immigration or unrelated background checks do not count

  • Can be done at the school (no need to go elsewhere)

⚠️ Submitting the application before fingerprints may cause delays.


STEP 1: Create Your Prometric Account

  1. Go to the Prometric CNA Portal

  2. Click Register a New User

  3. Enter:

    • First name

    • Email address (double-check spelling)

    • Password (must include):

      • At least 10 characters

      • Uppercase + lowercase letter

      • Number

      • Symbol

      • Not easily guessed

📌 Tip: Save your login info.


STEP 2: Start a New CNA Application

  1. Log in to Prometric

  2. Click Submit New Application

  3. Select:

    • Florida Certified Nursing Assistant Application

  4. Click Submit

✅ Always apply ONLINE
(Paper applications take longer and may get lost)


STEP 3: Fill Out Personal Information

Enter carefully:

  • Social Security Number (double-check)

  • Name (legal name)

  • Address

  • Phone number

  • Date of birth

  • Email address (most important – Prometric contacts you by email)

⚠️ Incorrect emails = missed exam dates.


STEP 4: Background & History Questions

  • Criminal history

  • Employment history

  • Medicaid fraud

  • Mental health questions

👉 Most students answer NO to all
👉 If YES, be honest

  • You are not automatically disqualified

  • May only need extra paperwork

  • Exemptions may be available


⭐ STEP 5: CERTIFICATION OPTION ELIGIBILITY

👉 SELECT: E4 (REQUIRED)

E4 – Training Center Information

  • THIS IS THE CORRECT OPTION

  • Leave Training Center Information blank

❌ Do NOT select:

  • E1 or E2 (traditional 120-hour schools)

  • E5 (not applicable)

ℹ️ E3 and E4 are similar, but E4 is preferred and recommended


STEP 6: Test Site Selection

  1. Choose Regional Exam Site

  2. Select:

    • Gabriel Health Institute (recommended)

📊 Students test better when they train and test in the same location


STEP 7: Select Exam Type

First-Time Testers (Most Students)

✔ Written Test + Clinical Skills
✔ Language: English
✔ Total Fee: $155

Optional:

  • Written Test in Spanish (if needed)

⚠️ Audio exam is not recommended unless absolutely necessary


Retesters (If Applicable)

  • Failed Skills only → Select Clinical Skills ($120)

  • Failed Written only → Select Written Test ($35)

  • Failed Both → Select Written + Skills ($155)


STEP 8: Payment

  • Select Credit Card

  • Avoid mail-in payments

  • Enter card details

  • Agree to terms

📌 SAVE THE APPLICATION halfway through

  • System may time out

  • Saving prevents starting over


STEP 9: Submit & Wait

  1. Click Submit

  2. Watch your email (within 1–2 days):

    • Payment confirmation

    • Application status updates

What Happens Next

  • Prometric verifies:

    • Fingerprints

    • Age (18+)

    • Valid SSN

    • No disqualifying felonies

  • Application sent to:

    • Florida Department of Health

    • Board of Nursing

  • Once approved:

    • Prometric schedules you for the next available exam date

IMPORTANT NOTES

  1. Gabriel Health Institute is the Regional Exam Site in Orlando. If you are taking classroom classes with us, or live in the Orlando metro area make sure to select “Gabriel Health Institute” as your Regional Exam Site. Student’s usually perform better when they Train and Test in the same location.
  2. Make sure you submit LiveScan Fingerprints and Background check screening, PRIOR to submitting the State Exam Application. Failure to do this will cause delay’s in the application process, and Prometric will NOT process your application until this pre-requisite is complete.

Welcome! The Florida Department of Health (DOH) has contracted with Prometric to develop and administer its Certified Nursing Assistant (CNA) Exam.

Here you will find all of the information you need regarding the CNA exam as well as important information on the CNA Registry.

APPLICATION PROCESS

The FL Department of Health/Board of Nursing and Prometric collaborate to ensure potential CNAs test as quickly as possible. There are several steps (outlined here) that must be completed by the CNA candidates, training programs, Prometric staff, and DOH/Board staff before a candidate can be deemed eligible and scheduled to test.

  1. Candidates or Training Programs: Submit complete testing application with testing fees to be received by Prometric at least 50 days before the requested test date.
  1. Prometric: Process applications within 3 business days and forward information to the DOH/Board.
  2. Candidates: Complete a Level II background screening with image capture.
  3. Board Staff: Review all submitted materials to determine candidate eligibility to test and notify Prometric of approval. (This step can take up to 30 days).
  4. Prometric: Schedule candidates upon receipt of eligibility from the Board. (Eligibility approval must be received from the Board at least 5 business days prior to the event or a change in test date may be required).
